Retiree form
As a courtesy, listing information provided by retirees is included each year in the printed version of the Student/Staff Directory. Please fill out this form if you are retired from the University of Illinois at Urbana-Champaign as of September 15. Please do not submit a form if you will be on the Urbana-Champaign payroll on that date; your listing will be automatically included in the directory.
A new form must be submitted each year. Retirees must submit a form by September 15 in order for their information to appear in the 2008-2009 Student/Staff Directory. Directories may be purchased from the Illini Union Bookstore beginning in November. Call 217-333-2050 to place an order or visit the booktore (map).
Allied and affiliated agency employee form
Employees who work for an agency that is allied or affiliated with the University of Illinois at Urbana-Champaign are responsible for providing the information that appears in the Student/Staff Directory. Those employees must complete this form each year in order to be included in the directory. Please do not submit a form if you will be on the Urbana-Champaign payroll as of September 15; your listing will be automatically included.

Suppression request form
Faculty and staff may withhold their home address and/or their home telephone number from the payroll records used to generate the printed directory listing by completing the online suppression request form by September 15.
Graduate students must complete this form and a Request to Suppress Directory Information Form from the Records Service Center, Admissions and Records Building, 901 W. Illinois, prior to the sixth day of instruction of any semester.
Office addresses and telephone numbers may not be suppressed because faculty and staff's employment with the university is a matter of public record. Suppression requests remain in effect until the faculty or staff member submits an online reinstate information form.

Reinstate information form
Complete the reinstatement form ONLY if you are a faculty or staff member AND you submitted an online Suppression Request Form in 2004. Completing this form indicates that you wish to have your home address and telephone number included in your listing information in the 2007-2008 Student/Staff Directory.
